Personalised care is about putting the person in the centre and recognising the value of the expertise each person gains from their own lived experience. With the support of health and care professionals, a person can be in the driving seat about what their own support should look like and what life outcomes they want to achieve. However, most people have others in their lives; friends, families and communities around them. Sometimes this means there are blurred boundaries between who is a “carer” and who is “cared for” – people mutually support one another. Therefore, Person centred care works best when professionals take a “whole family” approach. These illustrated stories take a look at some of the situations different families find themselves in and explore how a whole family approach is the most effective way to implement personalised care.
