Every young person deserves the chance to live a good life, with the right home, support, relationships and opportunities to be part of their community.
We can support you to help individual young people with complex support needs to create a clear vision for adult life.
By listening to the young person and the people who know and love them best, we build a practical plan based on what matters most to them, their strengths, aspirations and future goals.
The result is a personal life plan that helps shape decisions about housing, support, education, employment and community life.
A rights-based approach
Our approach is based on Article 19 of the United Nations Convention on the Rights of Persons with Disabilities (UNCRPD), which recognises the right to live independently and be included in the community.
We start with the belief that every young person should have the opportunity to:
- Have a home they can call their own.
- Build meaningful relationships.
- Be part of their local community.
- Make choices about their future.
- Receive the support they need to thrive.

Together we explore four key areas of life:
My life
Who I am, what matters to me, my strengths, hopes, dreams and important relationships.
My home
Where I want to live, who I want to live with and what I need to feel safe and at home.
My support
What good support looks like, what works for me and how people can help me achieve my goals.
My community
Friendships, activities, learning, work and the opportunities that help me belong and contribute.
Part of Brighter Futures
Brighter Futures is helping partners develop better opportunities for young people with complex support needs as they move into adulthood.
While the wider programme focuses on creating new local models of support, Individual Planning focuses on individual young people. It builds the foundations for a brighter future, one young person at a time, and helps turn aspirations into practical plans for adult life.
This focused individual approach is particularly relevant for young people at risk of admission to, or currently living in, inpatient settings.
