Parent & Carer Workshop 3, Time to Talk Next Steps
Thursday 07 April 2022, 19:00-21:00 | This session is only open to those taking part in the Time to Talk Next Steps programme.
Time to Talk Next Steps provides free support for young people aged 16-25 years in England with additional needs. In partnership with the National Development Team for Inclusion (NDTi), Contact are providing support, information and workshops for parents and carers about transition to adulthood.
Workshop 3: Health and Social Care (07 April 2022, 19:00-21:00)
- Using Person-centred planning to plan for transition to adulthood
- Adult Social Care – transition assessments and carers assessments
- Adult Health – including transferring from pediatric services to adult health teams, annual health checks, Continuing Health Care (CHC) funding and personal health budgets (PHBs)
